ABSTRACT

Objective:To explore the influencing factors of psychological stress in patients with coronary heart disease, and the effect path of joint decision-making between doctors and patients on psychological stress and to provide reference for the formulation of management plan for physical and mental health of patients with coronary heart disease.Methods:From July 2017 to April 2022, 715 retired patients with coronary heart disease hospitalized in General Hospital of the People′s Liberation Army and Air Force Hospital in Western Military Theater of the People′s Liberation Army were investigated adopted cross-sectional survey method by simple random sampling using general information questionnaire, Stress Response Questionnaire, Shared Decision-making Questionnaire between Doctors and Patients, Uncertainty in Illness Scale and Simple Coping Style Questionnaire. By using structural equation modeling, the direct and indirect influence paths of shared decision-making between doctors and patients on psychological stress were analyzed.Results:The scores of psychological stress, shared decision-making, uncertainty in illness, positive coping style and negative coping style were 49.81 ± 11.06, 79.24 ± 8.68, 76.40 ± 12.56, 21.67 ± 5.51 and 8.90 ± 3.12. Correlation analysis showed that the scores of psychological stress were positively correlated with the scores of uncertainty in illness and negative coping style ( r=0.661, 0.591, both P<0.01), and negatively correlated with the scores of shared decision-making between doctors and patients and positive coping style ( r=-0.623, -0.451, both P<0.01). Path analysis showed that shared decision-making between doctors and patients had a direct effect on psychological stress, accounting for 23.81% of the total effect; the uncertainty in illness, positive coping style and negative coping style played a part of intermediary role between the joint decision-making between doctors and patients and psychological stress, accounting for 32.35%, 8.40% and 17.93% of the total effect respectively; the uncertainty in illness and negative coping style had a chain mediating effect between them, accounting for 17.51% of the total effect. Conclusions:It suggests that the shared decision-making between doctors and patients with coronary heart disease is not only directly related to psychological stress, but also indirectly related to psychological stress through uncertainty in illness and coping style, among which coping style may play a role of "pivot" to a certain extent.

ABSTRACT

Objective:To investigate the application of system relaxation training on the posttraumatic stress disorder for the families of burned children, so as to provide a basis for the application of valid nursing in children families.Methods:A total of 80 cases of burned children in Children′s Hospital of Nanjing Medical University and corresponding 80 households were recruited and divided into the observation group and the control group with 40 cases each group. The patients and households from January 2019 to May 2019 in the control group received routine care, while the patients and households from February 2020 to June 2020 in the observation group received system relaxation training based on the routine care for four days. The posttraumatic stress disorder and uncertainty in illness of children families before and after intervention were evaluated by Posttraumatic stress Checklist-Civilian version(PLC-C) and Mishel Uncertainty in Illness Scale-Family Member form(MUIS-FM) and compared between the two groups.Results:There was no significant difference in the scores of PLC-C and MUIS-FM before intervention in the households between the two groups( P>0.05). After intervention, the scores of re-experiencing symptoms, avoidance/numbing, increased arousal symptoms and total PTSD scores in the households were (8.40 ± 1.79), (14.35 ± 2.85), (8.25 ± 1.28), (31.10 ± 3.52) points in the observation group and (11.28 ± 2.37), (16.75 ± 2.09), (9.10 ± 1.93), (37.13 ± 4.40) points in the control group, the differences were statistically significant ( t values were -6.76 - -2.32, all P<0.05). After intervention, the scores of ambiguity, deficit information and total MUIS-FM scores of the households were (37.08 ± 6.58), (20.15 ± 4.38), (84.38 ± 6.90) points in the observation group and (41.13 ± 6.54), (22.05 ± 3.32), (90.13 ± 7.85) points in the control group, the differences were statistically significant ( t =-2.76, -2.19, -3.48, all P<0.05). Conclusions:System relaxation training can alleviate posttraumatic stress disorder and reduce uncertainty in illness of the families of burned children.

ABSTRACT

Objective@#To investigate the current status of uncertainty in illness and caregiving burden in family members of patients with chronic wounds, and to analyze the relationship between them.@*Methods@#A total of 180 patients with chronic wounds admitted to the Department of Emergency of our hospital from October 2017 to March 2018, conforming to the study criteria, were selected by adopting the convenience sampling method. Then one family member who took care of the patients for the longest time and conformed to the study criteria were included in this cross-sectional survey. General Information Questionnaire made by the authors was conducted to investigate the demographic data and wounds of patients, and demographic data of family members. Chinese version of Parent Perception of Uncertainty Scale-Family Member (PPUS-FM) and Caregiver Burden Inventory (CBI) were used to investigate the uncertainty in illness and caregiving burden. Data were processed with multiple linear regression analysis and partial correlation analysis.@*Results@#The effective recovery rate of questionnaire was 91.7% (165/180). (1) The ages of patients were (71±17) years. Among them, there were 89 males and 76 females. The course of chronic wounds was 0.5 to 120.0 months. The wounds were mainly primary occurrence (86.1%, 142 patients), and the main type of wound was pressure ulcer (43.6%, 72 patients). Fifty-seven patients (34.5%) had wound infection. The ages of family members were (56±13) years, and 61.8% (102 people) of them were female. Their daily time of taking care of patients was (10±8) h. (2) The total scores of PPUS-FM of family members were 33 to 125 (88±17) points, mainly in medium level, in which the item score of unpredictability dimension was the highest. (3) The total scores of CBI of family members were 7 to 79 (43±14) points, in which the item score of time-dependence burden dimension was the highest. (4) Uncertainty in PPUS-FM could independently influence 10% of the total variation of caregiving burden in family members of patients (t=3.18, P<0.01). (5) The total scores of PPUS-FM of family members were in significantly positive correlation with the total scores of CBI and scores of physical burden, emotional burden, and social burden, respectively (r=0.33, 0.32, 0.25, 0.36, P<0.05 or P<0.01), while there was no obvious correlation between total scores of PPUS-FM of family members and time-dependence burden/developmental burden (r=0.14, 0.16, P>0.05).@*Conclusion@#There is positive correlation between uncertainty in illness and caregiving burden.

ABSTRACT

Objective@#To investigate the status of fatigue in maintenance hemodialysis (MHD) patients, analyze the correlation between fatigue and uncertainty and hope, and establish a multiple regression model to discuss the dependence between fatigue and uncertainty and hope situation in MHD Patients.@*Methods@#A convenient sampling method was used to select 150 patients with renal failure who were treated with MHD in the hemodialysis center of a third-class general hospital in Tianjin from June to September, 2018. The demographic data and disease data of the patients were collected with self-designed general data questionnaire. The patients were assessed with Piper Fatigue Revision Scale, Disease Uncertainty Scale and Herth Hope Scale.@*Results@#The results showed that the total fatigue score of MHD patients was 4.37 ± 1.17, belonging to the moderate level, with the highest score (4.95±1.35) in behavioral dimension, and the lowest score (3.48 ± 1.37) in cognitive dimension. The results showed that the score of fatigue were different in MHD patients of different ages, marital status, educational level, work status, primary disease, and the number of complications (P<0.05). The results of correlation analysis showed that the score of fatigue in MHD patients was positively correlated with the total score of uncertainty and its dimensions (r = 0.231-0.634, P<0.05). The fatigue level of MHD patients was negatively correlated with the total score of hope and its dimensions (r=-0.608- -0.323, P<0.05). Multiple linear regression showed that the independent variables entering the regression equation included marital status, number of complications, disease uncertainty and hope level score. And the determination coefficient R2 was 0.530,the adjusted determination coefficient was 0.501, F=18.454, P=0.000, which had statistical significance.@*Conclusions@#The fatigue level of MHD patients is moderate, and the score of fatigue in behavioral dimension is the highest, which is influenced by age, marital status, education, work condition, primary disease and the number of complications. The level of fatigue in MHD patients was positively correlated with disease uncertainty and its dimensions, and negatively correlated with the level of hope and its dimensions.

Objective@#To investigate the status of uncertainty in illness and its influencing factors among parents of children with Kawasaki disease complicated by coronary artery aneurysm.@*Methods@#A total of 102 parents of children with Kawasaki disease complicated by coronary artery aneurysms were investigated by General Information Questionnaire, Parent's Perception of Uncertainty Scale, Hospital Anxiety and Depression Scale.@*Results@#The total sore of Parent′s Perception of Uncertainty Scale was (91.02 ± 13.14) points; that of anxiety was (14.35 ± 3.93) points; while that of depression was (13.81 ± 3.92) points. The uncertainty in illness among parents was influenced by coronary artery aneurysm grading, source of medical expenses, education level, understanding of the disease, average family income (F=2.535-8.740, all P<0.05), relationship with the child, work status (t =2.057, -2.489, P<0.05). Multiple stepwise regression analysis showed that depression, understanding of the disease, source of medical expenses had a significant impact on the illness uncertainty of parents (F=13.695, P<0.05).@*Conclusions@#The uncertainty in illness among parents of children with Kawasaki disease complicated by coronary artery aneurysm had a higher level. The doctors and nurses should pay attention to the effect of uncertainty in illness on the parents of children and take targeted intervention measures to help parents reduce uncertainty in illness, thereby improving the quality of life.

ABSTRACT

Objective To research the illness uncertainty in people with pregnancy-induced hypertension and explore its influencing factors. Methods We investigated 150 cases of pregnancy-induced hypertension patients with Adaption of the General Information Questionaire, Mishel′ s Uncertainty in illness Scale, Social Support Rating Scale. Results The score of illness uncertainty was (66.79 ± 11.02) points. The illness uncertainty states for 76% of the patients was higher than normal. Multiple linear regression analysis showed that the severity of disease,degree of education,the number of pregnancy, subject support were major influencing factors. Conclusions There are higher level of illness uncertainty in the patients with pregnancy-induced hypertension.Medical workers should pay attention to the influencing factors of the illness uncertainty,and take action to lower the level of the illness uncertainty.

ABSTRACT

Objective To investigate the level of uncertainty in illness, anxiety and depression for mothers with premature infants in neonatal intensive care unit(NICU), and to analyze the correlation between them. Methods Totally 100 mothers with premature infants hospitalized in NICU were into the experiment group. They were investigated with the General Information Questionnaire, the Parent,s Perception Uncertainty Scale-Family (PPUS-FM), Self-Rating Anxiety Scale(SAS), and Self-Rating Depression Scale(SDS). Results The total score of uncertainty in illness was (82.840±29.132) points;the total score of anxiety was (43.360±13.033) points;the total score of depression was (60.740±16.847) points. The total score of uncertainty in illness, unclear, complexity, lack of information and unpredictability were positively correlated with the total score of anxiety and depression (r=0.186-0.399, P<0.05 or 0.01). Conclusions Mothers with premature infants hospitalized in NICU show a high level of uncertainty in illness, anxiety and depression, and they are interacted with each other. Therefore, in addition to nursing preterm infants, nursing staff should attach importance to their mothers′ psychological needs, help them reduce the uncertainty in illness so as to relieve the anxiety, depression and promote their health in body and mind.

ABSTRACT

Objective:To investigate the uncertainty in illness of patients with systemic lupus erythematosus (SLE) and to explore the influencing factors and humanistic care strategies.Methods:The general situation questionnaire and the uncertainty in illness scale were used to investigate the influencing factors of uncertainty in illness and positive humanistic care strategies in patients with SLE.Results:Patients with SLE were at a high level of uncertainty in illness,of which the unpredictability and uncertainty scored higher but lack of information and complexity were of lower scores.Educational status,family income,and self-perceived severity of disease were the main influencing factors of uncertainty in illness (P < 0.05).Conclusion:Patients with SLE were at a high level of uncertainty in illness,mainly from the unpredictable progress of disease and prognosis and unclear symptoms of the disease.Educational status,family income,and self-perceived severity of disease were the main influencing factors of uncertainty in illness.The clinical staff should pay attention to uncertainty in illness among patients with SLE and adopt positive humanistic care measures so as to reduce the sense of uncertainty in illness.

Conclusion Cognitive-behavior intervention for EMs patients can effectively decrease illness uncertainty, reduce negative emotion and improve coping capacity.

ABSTRACT

Objective To describe uncertainty in illness of patients with hepatocellular carcinoma intending to undergo transcatheter artery chemoembolization (TACE) and to identify the related factors. Methods A descriptive and correlative design was used. By convenience sampling, 95 patients with hepatocellular carcinoma were recruited from Sun Yat-Sen University Cancer Center in this study. Mishel Uncertainty in Illness Scale was used to assess uncertainty in illness and information needs of patients with hepatocellular carcinoma intending to undergo TACE and the demographic questionnaire and Information Needs Assessment Scale were used to measure demographic data, disease and treatment characteristics and information needs of patients. Results The level of uncertainty in illness in most patients with hepatocellular carcinoma intending to undergo TACE was middle (77.61±9.15) points. The mean score of indeterminacy subscale was (50.16 ± 6.16) points. Uncertainty in illness of patients with hepatocellular carcinoma intending to undergo TACE was affected by the degree of education background, domicile, family economic status, way to pay for the medical expenses, course of diseases, how many times he (she) used to undergo assisted treatments and the level of information needs. Conclusions The level of uncertainty in illness in most patients with hepatocellular carcinoma intending to undergo TACE was middle and it is important to assess patients′uncertainty in illness at clinical work and to take effective interventions to content patients′information needs to decrease the level of uncertainty.

ABSTRACT

Objective To investigate the level of illness uncertainty and coping styles of parents of children with diabetes mellitus and their relationship, and provide theoretical basis of clinical intervention. Methods Totally 155 patients of children with diabetes mellitus were investigated with General Information Questionnaire, Parents′ Perception of Uncertainty Scale (PPUS) and Coping Health Inventory for Parents (CHIP) to analyze the correlation between illness uncertainty of parents and its influencing factors and coping styles. Results The total score of illness uncertainty was 80.15±10.38 in parents of children with diabetes mellitus. There was significant difference in total uncertainty score in parents with different number of children, parent′s age, education degree, profession, economic conditions and locations (P < 0.05). Coping style, education and Parent′s age entered into the multiple linear regression equation of illness uncertainty, predicting 23.2% of total variation in illness uncertainty. The level of illness uncertainty was negatively associated with coping styles (P < 0.01). Conclusions The illness uncertainty was at higher levels in parents of children with diabetes mellitus. Clinical nurses should measure illness uncertainty of parents of children with diabetes mellitus and improve their coping capacity to reduce illness uncertainty, so as to provide better services of family and promote the quality of life for children .

Objective To explore the correlation of illness uncertainty and coping styles of patients with hepatitis B . Method A total of 254 patients with hepatitis B were assessed by Chinese version of Mishel′s uncertainty in illness scale ( MUIS ) and medical coping modes questionnaire ( MCMQ ) . Results The average score of illness uncertainty was at a higher level . The patients mainly used confrontation coping style. The illness uncertainty was positively correlated with the coping style of acceptance-resignation (P<0.05). Conclusion The illness uncertainty of patients with hepatitis B is at a higher level. Nursing intervention should be taken to reduce the degree of patients′uncertainty in illness and encourage them to take positive coping style.

ABSTRACT

Objective To investigate the influence of support and intervention technique with multi-dimensions on uncertainty in illness for cancer children′s parents. Methods With convenience sampling method, a total of 74 cases of hematological malignant and solid tumor patients′ parents in Hematology and Tumor Centre of Children′s Hospital of Fudan University were selected. There were 35 cases in the intervention group, which were given multi-dimension-support and intervention technique for 8 weeks as well as regular chemotherapy and nursing care. There were 39 cases in the control group, who were only given regular chemotherapy and nursing care. The level of uncertainty in illness in both groups were evaluated after 8 weeks of the base-line investigation. Results The scores of uncertainty in illness reduced over time in both groups (F=95.402,P<0.01). After 8 weeks, the scores of the intervention group were lower than those of the control group (F=24.079,P < 0.01). Conclusions The all-round, multi-dimension and real-time techniques of support and intervention could decrease the level of uncertainty in illness for cancer children′s parents.

ABSTRACT

Objective To explore the correlation between the cancer-related fatigue and uncertainty in illness among breast cancer inpatients. Methods Totally 90 patients with breast cancer were involved in the study by cancer fatigue scale (CRS) and Mishel's uncertainty in illness scale-adult (MUIS-A). The correlations were analyzed. Results Approximately 100.00%patients experienced fatigue during hospitalization. The total score of uncertainty in illness was 107.51 ± 21.26. There was a positive correlation between each fatigue dimension and uncertainty in illness (P<0.001). Conclusions Cancer-related fatigue is widespread among breast cancer inpatients and sense of uncertainty in illness of patients is at a moderate level. Nursing staff can help patients to reduce disease uncertainty and improve the life quality of patients by strengthening health education , providing comprehensive information support, fully mobilizing patients' social support system.

Objective To investigate the status of uncertainty in illness and social support among patients with post-traumatic brain syndrome (PTBS) and analyze their correlation.Methods One hundred and fourteen patients with PTBS were selected by convenience sampling and assessment.They were evaluated by Chinese disease Mishel Uncertainty in Illness Scale (MUIS) and Social Support Assessment Scale (SSAS) and the correlation of two scales was analyzed.Results MUIS total score was(135.23±19.29) scores with a median of 128.74 scores and SSAS total score was (43.14±5.54) scores with a median of 42.00 scores in patients with PTBS.Among 114 patients with PTBS,regarding ambiguity score was negatively correlated with objective support,subjective support,utilization degree of support and SSAS total score,r=-0.301,-0.374,-0.521,-0.509,complexity score was negatively correlated with objective support,sub-jective support,SSAS total score and utilization degree of support,r=-0.367,-0.326,-0.355,-0.533,both lack of information and regarding unpredictability score were negatively correlated with utilization degree of support,r=-0.376,-0.330,MUIS total score was negatively correlated with objective support,utilization degree of support and SSAS total score,r=-0.284,-0.424,-0.305.Conclusion There is high uncertainty of PTBS patients;increase of patients' social support will be good for decreasing the ambiguity and com-plexity among patients' uncertainty;promotion of patients' utilization degree of support will help to reduce patients' lack of information and unpredictability.

ABSTRACT

Objective To explore the influence of uncertainty in illness in compliance of respiratory function exer-cise and rehabilitation after surgery in patients with coronary artery bypass graft (CABG) surgery. Methods Mishel uncer-tainty in illness scale and compliance of respiratory function exercise scale were used to collect data in 108 patients with CABG. The correlation of the uncertainty in illness and compliance of respiratory function exercise was analyzed. Patients were divided into three groups (high score, medium score and low score groups) according to the detection of uncertainty in illness. Values of compliance of respiratory function exercise, postoperative care time, postoperative ventilation time, postop-erative hospital stay duration, 6-min walking test (6MWD) and left ventricular ejection fraction (LVEF) were compared be-tween three groups. Results The uncertainty in illness was in medium level (95.51±9.48) in patients with CABG surgery, which was negatively correlated with compliance of respiratory function exercise (r=-0.926, P<0.01). The postoperative venti-lation time and postoperative care time were less in low score group than those of medium score group and high score group. The compliance of respiratory function exercise, 6-min walking test and postoperative LVEF were better in low score group than those of medium score group and high score group. Conclusion Medical personnel should take targeted interventions to reduce the uncertainty in illness in patients, enhance the compliance of respiratory function exercise, reduce the postopera-tive complications and promote early recovery of patients.

ABSTRACT

Objective To investigate the current status of uncertainty in illness in cardiac valvular surgery patients with cardiopulmonary bypass and analyze its influential factor,and provide reference for effective intervention.Methods A total of 208 cardiac valvular surgery patients with cardiopulmonary bypass were recruited from 4 cardiovascular surgery in Guangdong and were investigated with the self-designed questionnaire,Mishel Uncertainty in Illness Scale (MUIS),Connor-Davidson Resilience Scale (CD-RISC),Medical Coping Modes Questionnaire (MCMQ),in the preoperative day and the day before discharge.The data were analyzed by single-factor and multiple-factor analysis.Results The average score of uncertainty in illness in cardiac valvular surgery patients with cardiopulmonary bypass was 99.09±11.41,and 92.3% of patients got a moderate uncertainty in illness level in the preoperative day.The average score of uncertainty in illness in the patients was 90.33± 10.32,and 91.8% of patients got a moderate uncertainty in illness level in the day before discharge.Multiple linear regression indicated that,age,cardiac valvular disease types,number of cardiac valvular surgery,optimism and yield were the protective factors of uncertainty in illness level.Average monthly household income,self-improvement,scores of resilience and confront coping were the risk factors of uncertainty in illness level.Conclusion The level of uncertainty in illness in cardiac valvular surgery patients with cardiopulmonary bypass is moderate.Age,average monthly household income,cardiac valvular disease types,number of cardiac valvular surgery,resilience and coping style are factors influencing uncertainty in illness level of patients.
